Behaviorists often apply their practices in schools and institutions where they have more control over outcomes and the environment. Obviously, if a patient does not cooperate with treatment while in his/her social milieu, treatment will not be effective, but doesn't that say the person does not want help, at least in this way? This is always a problem. Usually, behaviorists try to control this problem by teaching and reinforcing behaviors that replace or are incompatible with the maladaptive behaviors you want to eliminate. You don't just eliminate one behavior and hope that they learn an adaptive behavior. That is a prescription for disaster.
